During our examination, we ruled out intermittent claudication, a condition associated with Peripheral Artery Disease (PAD) in which there is cramping pain in the calf and leg muscles during exercise and relieved by rest. During Oliver&#8217;s exam we noted in both legs the presence of varicose veins, edema, skin thickening, a brownish discoloration and a draining leg ulcer in his left leg.</p><p style="text-align: justify;" align="justify">Oliver&#8217;s history and physical  suggested strongly that his vein problem was secondary to an underlying condition caused by damaged one-way valves in his deep veins called <a title="chronic venous insufficiency treatment" href="http://www.jvai.com/vein-treatment-disorders/chronic-venous-insufficiency/">chronic venous insufficiency or CVI</a>. We performed a highly specific ultrasound examination that confirmed the exact pathology causing Oliver&#8217;s vein disease problem. Without other symptoms, they are considered to be a      cosmetic problem.</li><li style="text-align: justify;"><strong>Varicose veins</strong> &#8211; these twisted, distended veins due to      increased pressure, affect larger surface veins in the legs. The three      primary areas of venous incompetence involve the Greater Saphenous Vein on      the inside of the thigh and calf, the Small Saphenous Vein on the back of      the calf and the Perforator veins of the calf.</li><li style="text-align: justify;"><strong>Phlebitis or superficial venous thrombosis </strong>- this      refers to a blood clot that forms in a vein close to the skin&#8217;s surface.      These clots cause pain but generally do not travel to the lungs.</li><li style="text-align: justify;"><strong>Deep vein thrombosis </strong>- also called a DVT, this is a clot in the deep, large veins of the legs. It can become life-threatening if the clot dislodges and travels to the lungs. (pulmonary embolism).</li><li style="text-align: justify;"><strong>Venous leg ulcers</strong> &#8211; commonly found below the knee and      above the ankle on the inner part of the leg, these wounds or open sores      occur from static blood flow and are resistant to healing. They are also      called venous stasis ulcers.</li></ul><p style="text-align: justify;">Patients with venous disease have many signs and symptoms that compromise their quality of life. Untreated venous disease can progress to more serious conditions. Regardless of the stage or type of venous disease, office vascular surgery treatments at JVAI can cure these conditions and prevent their progression.</p></div><h4><a href='#' alt='What causes venous disease?' >What causes venous disease?</a></h4><div class='faq-answer'><p style="text-align: justify;">Most vein problems are due to increased pressure in the venous  system. This is actually high blood pressure of the veins located under  the skin. This increased pressure is caused by malfunction of the valves  that lie within the veins, causing the veins to dilate. When the veins  become dilated, the valves, which are attached to the sidewalls, are  pulled apart. They no longer are able to maintain one-way flow and  prevent blood from draining back toward the feet. These leaking veins  are called incompetent.</p><p>Risk factors may predispose an individual to develop venous disease. Distention of the veins over time produces  varicose veins. It also increases the pressure in the smaller veins  causing these to stretch and become very prominent. When these are just  under the skin, they are known as spider veins. The increase pressure  causes the small veins to leak fluid into the surrounding tissues  resulting in swelling or edema followed by discoloration. The high  pressure is also transmitted to the smallest veins that lie near the  capillary beds. This makes it difficult for blood to flow across the  smallest vessels or capillaries, thus disrupting the exchange of  nutrients and wastes in this area. The tissues of the leg respond by  thickening of the skin and finally by ulceration. A serious consequence  of this type of vascular disease is Infection of the skin or cellulitis.</p></div><h4><a href='#' alt='What is chronic venous insufficiency?' >What is chronic venous insufficiency?</a></h4><div class='faq-answer'><p style="text-align: justify;">Chronic venous insufficiency or CVI occurs when one-way valves in the veins that normally keep blood from flowing backwards, become damaged or incompetent. Originally in 1998, he pioneered  the Subfascial Endoscopic  Perforator Surgery or SEPS procedure for the treatment of incompetent perforator veins,   performing one of the highest  volumes of SEPS procedures and achieving   over a 96% cure rate of  venous ulcers.</p><p style="text-align: justify;">Subfascial Endoscopic  Perforator Surgery or SEPS is used to correct incompetence of the venous perforator system—a major cause of severe venous disease and leg ulcers. Many patients with  ulcerations of the ankle were cured through the SEPS procedure. This  minimally invasive procedure is performed utilizing a small television  camera system that is introduced into the leg in an outpatient surgery  setting. Small titanium clips are placed to close off the leaking  perforator veins. The entire procedure is performed through a 1-inch  incision just below the knee. Correction of this problem reduces leg  swelling and significantly aids in the healing of chronic leg ulcers. Dr. Joyce performed over 600 of these surgeries over a three year period, one of  the highest volumes in the country. The results were excellent  with 94% of patients healing their ulcers.</p><p style="text-align: justify;">Although  SEPS worked well with excellent results, it required  hospitalization and  general anesthesia which was expensive and inconvenient for patients. Dr. Joyce made it his mission  to provide the same or an improved  treatment outcome through  procedures that could be performed within the  office setting using  local anesthesia. He began to pioneer  two new less  invasive technologies and within a short period of time,  totally  replaced SEPS with VNUS radiofrequency closure and laser ablation. An ultrasound confirmed incompetence in her venous perforator and saphenous vein systems. Her varicose vein treatment corrected these problems during office procedures in which she was awake and comfortable. Nancy readily recovered with a complete elimination of her spider veins, varicose veins, venous disease and associated symptoms.</p><h3>How Venous Disease Begins</h3><p style="text-align: justify;" align="center">Two systems of veins in the legs, the surface or superficial veins and the deep veins, allow for the proper movement of blood back to the heart. The superficial veins lie just under the skin. The deep veins lie within the large muscles of the leg. The two vein systems are connected by short veins called perforator veins. All of the veins have valves to maintain one way flow. When you walk or exercise, your calf muscles squeeze large deep veins and pump blood out of your legs towards your heart.</p><p style="text-align: justify;" align="center">With proper function of the valves, the venous system of your legs is designed to overcome the effects of gravity and pump blood out of your legs towards your heart where it picks up a fresh supply of oxygen. When valves in the superficial and deep veins become incompetent, gravity reverses the flow of blood back towards the feet. Ultimately, this produces an increased blood pressure in the surface veins of the leg. Incompetent veins in the superficial and deep veins can produce spider veins, varicose veins, leg swelling, leg ulcers and serious infection.</p><h3>Signs of Venous Disease</h3><p style="text-align: justify;" align="center">One of the first signs of venous disease and the need for vein treatment is distention of the delicate superficial skin veins or <a title="spider veins" href="http://jvai.com/vein-treatment-disorders/spider-veins/">spider veins</a>. With progression of the disease, there is dilation of the larger veins resulting in <a title="varicose veins" href="http://jvai.com/vein-treatment-disorders/varicose-veins/">varicose veins</a>. Pressure from distended larger veins on nerves results in aching pain and night cramps. Often patients report unusual sensations described as “water flowing down the legs” or “crawling feelings on the skin.” Swelling or edema occurs from a leakage of water into tissues from the dilated veins. Blood may also leak out as the pressure increases. This produces a brownish skin discoloration. The disease compromises the normal exchange of nutrients and wastes at the level of the capillary beds. This causes a thickening of the skin and/or death of tissue resulting in a <a title="venous leg ulcer" href="http://jvai.com/vein-treatment-disorders/venous-leg-ulcers/">venous leg ulcer</a>. Another serious consequence of this type of venous disease is infection of the skin or cellulitis.</p><p style="text-align: justify;" align="center">Sometimes venous disease begins with a traumatic leg injury or a spider bite that is resistant to healing in patients as young as their teens. Patients may attribute their tired, aching legs, exercise intolerance and other symptoms to an injury or just to getting older. The result is often years of misdiagnosis and mismanagement with one treatment failure after another. He consulted three physicians, was treated for wound care and declined a vein stripping procedure by a vein surgeon in Georgia who gave him only a 50% chance for success. The problem tremendously impacted his life causing unbearable symptoms that interfered with his ability to work and sleep. The patient’s mother, from Port Charlotte, FL read about Douglas H. Joyce, DO, founder of the Joyce Vein &amp; Aesthetic Institute, in the Florida Health Care News. Shortly after, he made arrangement to travel to Florida for treatment.</p><p style="text-align: justify;">The patient reported no remarkable health history and presented for treatment of his severe venous disease with ulceration in his left leg. His symptoms included left lower extremity prominent painful bulging subcutaneous varicose veins with edema (swelling), discolored thickened skin with inflammatory dermatitis and ulceration. He complained of a heavy sensation in his legs, cramping, painful, tender and swollen legs. He had used support hose for greater than six months and elevated his legs whenever possible. His condition interfered with normal daily activities, producing absences from work and sleep disruption due to pain.</p><p style="text-align: justify;">The patient was found to have venous disease of his saphenous and perforator systems and underwent laser ablation procedures at JVAI. Treatment at JVAI resulted in total healing of the leg ulcer and a complete reversal of the patient’s Venous Insufficiency in his left leg. &#8211;Douglas H.
