What can I expect during the hair removal procedure?

John’s Venous Leg Ulcers Healed in 3 Weeks after Treatment at JVAI

Healing leg ulcers and eliminating the reason for his existing chronic venous insufficiency ended years of misery and suffering for this 48 year old man.

John R., a 48 year old white male came to us with severe ulcerations in his left leg, along with swelling, skin thickening and pronounced discoloration in both legs. He reported a previous ulcer that had healed. John complained of painful, tender legs with aching and leg cramps – symptoms that disrupted his normal activities, caused him to be absent from work and disturbed his sleep. He relied heavily on pain and anti-inflammatory medication for his varicose veins. His venous disease continued to progress despite wearing medical grade compression stockings and elevating his legs whenever possible for a period greater than three months. Family history was positive for venous disease. On complete venous ultrasound examination, extensive disease of John’s great saphenous, small saphenous and perforator vein systems was detected in both legs. He was diagnosed with chronic venous insufficiency, severe hemosiderosis, moderate lipodermatosclerosis, varicose veins and ulceration on his left anterior and medial calf. He underwent multiple laser ablation procedures in our state-of-the-art vein treatment center to correct all areas of venous disease. All leg ulcers completely healed within three weeks. His severe discoloration is expected to fade considerably over the next several months.

John R’s underlying condition was due to incompetent one-way valves in his superficial vein system called chronic venous insufficiency or CVI. Our venous ultrasound evaluation that we used to map his pathology, allowed us to accurately define all diseased venous areas in need of correction. During John’s saphenous and perforator laser ablation procedures in our office, he remained awake and comfortable with the use of local anesthesia. Each minimally invasive surgery involved the introduction of a laser through a single needle stick while we corrected abnormalities that were visualized on a monitor. John was able to immediately walk out of the operating room and resume light activities. Within a few days, he was able to return to work and over the next few weeks, his symptoms continued to improve. Within three weeks, all of his ulcers had healed.

It is a fact that quality sleep is essential for good health. Without restful deep sleep, the body’s ability to restore, recover and re-energize is greatly reduced. This compromises the immune system and puts into motion, a cycle of degenerative disease that can produce chronic and life threatening illnesses as well as premature aging.

We have all suffered from the effects of an occasional night of sleeplessness but for those who lie awake night after night, tossing and turning from aching leg pain, night cramps or unusual sensations like crawling feelings on their skin or water flowing down their legs, life can become unbearable.

Millions of Americans suffering from restless leg syndrome are prescribed drugs to suppress these symptoms. For those whose symptoms are not related to a treatable condition, prescription medications can greatly enhance sleep as disturbing sensations and movements in legs are quieted. Unfortunately, medications may be prescribed without a proper evaluation to rule out and eliminate treatable causes of restless legs. Sometimes the cause of restless legs is a neurological disorder or a nutritional deficiency and sometimes it is related to a curable disease of the veins. Unfortunately, when the problem is masked by prescription drugs, an underlying disease like incompetent valves in veins, can progress and become a major health issue.

Surprisingly, many physicians are not aware of the possibility that restless legs can actually be caused by venous disease – a serious condition that increases risks for bleeding, blood clots, leg ulcers and a progressive poor quality of life. Even when there are no obvious signs of a venous disorder such as bulging varicose veins, leg swelling, dark thickened discolored skin and/or leg ulcers, pathology in the valves of the superficial and deeper vein systems can be present and produce restless leg symptoms during times of rest and sleep.

How Venous Disease can Mimic Restless Legs

Normally, the venous system is well equipped to move blood back to the heart by pumping blood out of the legs against gravity through muscular contractions. Valves in the veins are designed to prevent blood from leaking backwards into the legs. When valves in the veins of the superficial or deeper vein systems become incompetent, blood flow reverses and produces increased blood pressure (venous hypertension) in the affected blood vessels that expand to accommodate the enlarged blood volume. Depending upon the location of the problem and severity, patients can suffer from a range of symptoms in the surface veins and surrounding skin. Obvious signs of a venous disorder include spider veins, varicose veins, leg swelling, leg ulcers and serious infection. But venous disease in the deeper vein system can be present with less obvious symptoms. Patients may have less specific complaints such as leg cramps at night, leg pain, restless legs, poor sleep, poor energy during the day, difficulty standing for long periods and problems walking even short distances.

Evaluation by a Vein Treatment Specialist

Anyone suffering from restless legs, leg cramps at night, leg aching or pain, swelling or intolerance to standing and walking should be evaluated for a possible venous disease problem. A vein doctor who is a vein treatment specialist (board certified phlebologist) is the only practitioner who is properly trained and equipped to accurately diagnose and treat a venous disorder. The evaluation or vein screening will be focused on determining signs and symptoms of venous disease, family history, medical history and previous medical and surgical treatments. All of this information in combination with a specialized ultrasound exam, will be used to determine whether a vein problem exists and the most appropriate plan for eliminating the disease.

Important medical history:

Without any obvious vein symptoms, patients with complaints of restless legs, itchy skin on the legs and occasional swelling can be significant. The patient’s history may include problems that put them at higher risk for vein diseases. Patients with leg trauma and/or a history of phlebitis and DVT for example, are at higher risk for developing venous disease. Part of the history involves ruling out a condition called intermittent claudication associated with PAD or Peripheral Arterial Disease.

Physical exam:

The history is followed by a physical exam to detect obvious signs of a venous problem: spider and varicose veins, leg swelling, leathery discolored skin and ulceration.

Diagnostic exam:

Following the history and physical exam, a diagnostic venous ultrasound exam is performed for detecting venous pathology in the superficial and deep venous systems. With positive findings, an appropriate vein treatment plan is determined.
