State-of-the-Art Treatment Offers Cure for Venous Diseases

What is Venous Disease?
High blood pressure of the surface veins of the legs produces major negative changes. It causes the veins to distend over time resulting in varicose veins. It also increases the pressure in the smaller veins causing these to stretch and become very prominent. When these are just under the skin, they are known as spider veins. The increased pressure causes the small veins to leak fluid into the surrounding tissues resulting in swelling or edema followed by discoloration. The high pressure is also transmitted to the smallest veins that lie near the capillary beds. This makes it difficult for blood to flow across the smallest vessels or capillaries, thus disrupting the exchange of nutrients and wastes in this area. The tissues of the leg respond by thickening of the skin and finally by ulceration. Infection of the skin or cellulitis often occurs. Leg Ulcer Treatment at JVAI
How are Venous Diseases Treated?
The treatment of venous disorders of the leg centers on the correction of the underlying problem - high venous blood pressure. The two areas of primary interest are incompetence of the greater saphenous vein that lies in the thigh, and correction of incompetent perforator veins of the calf. Incompetence of the greater saphenous vein has traditionally been treated by vein stripping. In this procedure, an incision is made in the groin and the greater saphenous vein is tied off and divided. An instrument called a vein stripper is then inserted into the vein and the vein is actually torn out of the leg. Pressure is applied afterwards to control bleeding from any branches. This procedure has been used for decades and produces good results. However, it is painful and has a prolonged recovery period.
Venous Disease
As a board certified vascular, cardiac, and thoracic surgeon, I identified numerous patients that were suffering from venous diseases. Many were erroneously diagnosed with arterial disease and referred to me for treatment. In September of 2001, I opened the Center for Circulatory Health & Vein Treatment Center in Port Charlotte, Florida (now the Joyce Vein & Aesthetic Institute in Punta Gorda, Florida). My interest in vein treatment was prompted by the discovery of safe, minimally invasive treatment that actually corrected the underlying problem. This alternative to traditional invasive painful treatment eliminated high venous blood pressure in just a few visits.
ELAS: JVAI’s Solution for Varicose Veins
At the Joyce Vein & Aesthetic Institute, vein stripping is NEVER done. Instead a procedure known as endoluminal laser ablation of the saphenous vein or ELAS is used. In this procedure, a fiber-optic laser system is inserted into the greater saphenous vein through a small needle puncture. The greater saphenous vein is then ‘welded shut’ from the inside by the laser system. The procedure generally takes under 1 hour and the patients are able to leave the office immediately.

Large numbers of patients have had excellent results with ELAS. Many of these patients had undergone vein stripping in the past and then presented for endoluminal laser surgery on the other leg. All of these patients have felt that there was no comparison between the two procedures. The laser ablation was less painful and had a shorter recovery by far. Incompetence of the venous perforators is corrected by subfascial endoscopic perforator surgery or the SEPS procedure. Many patients with ulcerations of the ankle have been cured by this procedure. This minimally invasive procedure is performed utilizing a small television camera system that is introduced into the leg in an outpatient surgery setting. Small titanium clips are placed to close off the leaking perforator veins. The entire procedure is performed through a 1-inch incision just below the knee. Correction of this problem reduces leg swelling and significantly aids in the healing of chronic leg ulcers. I have performed over 600 of these surgeries over the past 3 years, one of the highest volumes in the country. The results have been excellent with 94% of patients healing their ulcers.


Varicose veins are removed by Microphlebectomy. This European technique involves removing the prominent veins through tiny needle punctures, virtually leaving no scars. It is quick and relatively painless and produces superior results than traditional treatment. My clientele includes patients of all ages, both male and female. I have treated a number of young women who are looking for a superior cosmetic result such as aerobic instructors and models. Sclerotherapy is also utilized for smaller varicose veins when indicated.
